Whether you are a new member or an existing IqHS member, you are invited to complete this registration form so that we can keep your personal profile information in our database for future use, communications and participation in our activities, scientific meetings, conferences, news letters, research, etc. We would also like to inform our dear members that the IqHS shall not share any information with others and for whatever reason, and their personal information are regarded as confidential.

N.B. For the purpose of issuing an IqHS ID card, please fill out the form in English exclusively except the full name which will be in Arabic as well and upload a photo of yourself against a white background.
